Docs: rewrite CLAUDE.md (202→56 lines), server/CLAUDE.md (212→76), create contexts/server-operations.md, fix .js→.ts references, fix env var prefix in README, rewrite INSTALLATION.md, add CONTRIBUTING.md and .env.example.
